Job Summary:
We are currently seeking a Care manager to join a Housing and Community Care team in North West London
Responsibilities:
To receive allocated work under the direction of the Team Manager or Senior Practitioner.
To ensure a needs led approach in the assessment of service user needs and promote independence.
To assess carers’ needs.
To apply Fair Access criteria in determining eligibility for services, and assist service users in gaining access to appropriate resources and services.
To ensure that proposed care packages represent best value and quality, and that service user financial contributions are assessed.
To review care packages.
To participate in regular supervision with the Team Manager or Senior Practitioner in line with the departmental supervision policy.
To be appraised by the Team Manager or Senior Practitioner in line with the corporate performance management scheme.
To identify learning and development needs, participate in agreed learning opportunities, and implement learning experiences in practice.
To be committed to continuously improving quality, and supporting the unit to meet performance standards.
Pay rate is between £20 – £25 per hour
GSCC Registration is essential.
