Job Summary: We are currently seeking a Reablement Senior Practitioner for a position in South East London Responsibilities: To co-ordinate a Reablement Team (5 Reablement Assessors, 1 admin worker) to deliver a Reablement Service in Southwark Adult Social Care. To maintain the team to implement the principles of Reablement as a service. To devise Reablement service rotas to ensure effective running of the day to day service To work with the Team manager to establish clear processes and pathways to ensure that the pilot is delivered in a timely and efficient manner. To collect and collate statistical information to demonstrate the outcomes and learning from the pilot to the Project Personalisation Board. To participate in Operational Team Management Meetings when required. To operational responsibility of delivery a 5 day a week service To have an operational overview of the service across Southwark and to control admission to the service according to the agreed resources provided by the agencies involved. To be involved in training the Reablement Support Workers to provide a ‘to do with not for’ approach to care. To develop with the Team manager operational processes in which to deliver the pilot objectives. • Pay rate is between £25– £30 per hour GSCC Registration is essential.
