Job Summary:
We are currently seeking a Senior Care Manager to join an Adults team in South East London
Responsibilities:
Assess the needs of individuals and their carers.
Together with the service users and carers, formulate and implement care plans appropriate to their needs.
Provide counseling and continuing support to users and carers where appropriate.
Monitor, review and evaluate services in order to ensure that they are provided effectively and in accordance with need.
Manage, supervise and support staff as appropriate.
Assist and participate in the management of the Duty service and act as Duty Senior.
Provide an assessment service to individuals, their carers and other agencies in accordance with fair access to care eligibility criteria. These cases are likely to fall within the critical and substantial bandings.
Initiate and co-ordinate care plans for and with individual service users and their carers, in conjunction with professional colleagues and representatives of voluntary and private agencies.
As necessary, ensure that work is allocated, transferred or concluded, in accordance with Divisional criteria.
Offer support and advice as appropriate to service users and their carers.
• Pay rate is between £24– £28 per hour
GSCC Registration is essential.
